FOR.MO Survey SaaS (v1.0.0) - cPanel install

1) Upload & Extract
- Upload ZIP contents into the document root for https://for.mo.sg
- Files must be in domain root (NOT /forms)

2) Create MySQL database
- Create DB + user in cPanel
- Grant ALL privileges

3) Configure
- Edit config/config.php:
  - db.dsn / db.user / db.pass
  - app.url = https://for.mo.sg
  - PayNow QR path: public/assets/paynow.png
  - Optional: enable Stripe + add price IDs

4) Install
- Visit: https://for.mo.sg/install.php
- Create first system admin
- DELETE install.php after success

- Checkout is included
- Webhook auto-confirm is NOT included (per request)
